Too often, the future we envision is created using current concepts instead of re-imagining new concepts. We apply the priorities and relative positions of key players as they are today. Assumptions about what is possible become nothing more than extensions of what is currently possible, and not what can be possible. For short periods of stability that may be good enough.  However, this is a period of instability created by the false economy of health care. 
Health costs are much higher than what they should be. From purchaser to patient it is not clear what anyone is really buying. The American health care system looks more like a  planned economy than a vibrant competitive market. 
The times are ripe for disruptive change. New possibilities will change the way we view the world. Core realities and internal forces are erupting and will alter the landscape.
